You must first complete and fulfill all mission requirements on all stages, including the Emergency Stage 6 boss rush without dying and the final level within the time limit. This stage is quite a jump in difficulty compared to the rest of the game, as you'll be finding yourself dodging ridiculous amounts of bullets and enemies almost non-stop. If you're having difficulty with the boss rush, you can use 2 players, as it will not disqualify you if either of you die.
